A Fresh Start
When P (person supported’s name changed for anonymity) first moved to Gilburn four years ago, they were transitioning from a hospital setting where they had lived for two to three years. Understandably, the change came with uncertainty – but it also opened the door to new beginnings.
“When I first came before I moved in, I had three team members each day – now two. I don’t need as much support now as I did when I first came here.”
From the start, the Gilburn team worked closely with them to create a personalised plan of care and support, focusing on stability, trust, and gradual independence. Each step was taken at their own pace, with the person actively involved in decision-making about their daily life and goals.
Building Skills and Confidence
Over time, P began developing independent living skills – from cooking and cleaning to planning community activities and holidays.
“I have my own apartment, and I’m able to have privacy and my own time should I want it. My apartment is a good size, and I like it.”
The support team helped P build confidence in completing everyday tasks, while also encouraging social engagement and community participation.
“I get support to go out and do things I want to do, and I’m planning a holiday later in the year.”
These achievements represent more than practical milestones – they’re markers of personal growth, self-trust, and belonging.

At the heart of Gilburn’s success is its warm, person-centred culture. P describes strong relationships with both their key workers and the Home Manager, Heidi, who ensures every person feels heard and supported:
“I like the manager, Heidi – I think she is pretty cool! I give her 50 stars, and I can come and talk to her when I want. She meets with me monthly, and I can talk about whatever I want with her.”
Having consistent, trusted relationships has helped P settle, adapt to new team members, and stay motivated to continue their journey.
“I have two key workers, and I got to pick who they are. They do shifts with me and support me to organise trips and give me someone to talk to.”
A Home Built Around the People We Support
Gilburn offers a modern, homely environment designed to empower people with learning disabilities, autism spectrum conditions, and mental health needs. Each apartment is fully personalised, allowing people supported to enjoy privacy and comfort while having access to shared spaces and community activities.
Registered Manager Heidi explains:
“I started working in care in my 20s and have supported both adults and children with learning disabilities, autism, and mental health needs. I do this job because I love it – helping people progress and achieve positive outcomes in their lives.”
This passion is reflected in the team’s everyday work – supporting individuals to take brave steps forward, no matter how small.

Today, P’s life looks very different from when they first arrived. They live in their own apartment, manage many of their daily tasks independently, and are planning their next adventure – a well-earned holiday.
